Basix Total Optimization Program Our Basix Total Optimization Program combines both metabolic and hormone optimization to address the underlying factors that influence energy, metabolism, body composition, and overall well-being. The Comprehensive Health Optimization Program includes: • Advanced metabolic and hormone laboratory testing • Detailed provider consultation and lab review • Personalized treatment plan based on your results and goals • Medical weight optimization (price includes cost of weight loss medication) • Medication management and treatment adjustments • Ongoing monitoring and follow-up visits This program allows us to evaluate the full picture of your health and create a personalized strategy to improve energy, metabolic function, and overall wellness. Who This Program Is For This program may be beneficial for individuals experiencing: • difficulty losing weight • persistent fatigue or low energy • hormone imbalance symptoms • brain fog or difficulty concentrating • metabolic concerns such as insulin resistance • reduced vitality or overall wellness Program Structure $299 Basix Assessment (Applied toward your program if you enroll) Program Length: 6 months Includes: comprehensive baseline, 3-month, and 6-month laboratory assessments; monthly email check-ins for ongoing support; two scheduled telehealth follow-up visits; Cost of GLP-1 medication therapy; and personalized hormone replacement, hair loss prevention, and sexual health prescriptions (if indicated) sent to your preferred pharmacy—along with continuous medical management to optimize your results. Cancellation Policy
FINANCIAL RESPONSIBILITY & PAYMENT POLICY Basix Health & Wellness is a self-pay practice and does not bill insurance. You are financially responsible for all charges associated with your care. Fees are disclosed before services and may change with notice. Payment is due at the time services are rendered unless otherwise agreed. Missed Appointments / Cancellations: At Basix, appointment times are reserved specifically for you. To respect the time of our providers and other patients, we ask that all schedule changes be made in advance. Cancellation & Rescheduling Appointments may be cancelled or rescheduled up to 24 hours before your scheduled visit without penalty. Changes made with less than 24 hours’ notice may be subject to a late cancellation fee of $25 No-Show Policy Patients who miss an appointment without notice may be charged a no-show fee of $25. Repeated missed appointments may require prepayment for future visits. Late Arrivals Patients arriving more than 10 minutes late may need to reschedule to ensure adequate visit time. Membership / Program Patients Coaching or follow-up sessions included in programs must be cancelled at least 24 hours in advance or the session may be forfeited. Refund Policy: Fees for services already rendered are non-refundable. Program fees, if applicable, are governed by the terms disclosed at enrollment.
